  1. Select the printable form if you would would like to pay by check, UB Payroll deduction, or to pay by credit card offline.
  2. Minimum $10 gift amount. Charge will occur around the 15th of each month and will recur until cancelled.
  3. Minimum $100 gift amount with minimum $50 installment amount. Gift amount must be evenly divisible by installment amount. Charge will occur around the 15th of each month until your total gift amount is paid.
